To Make an Appointment
Appointments can be made for diabetes education consultations with a nutritionist, exercise physiologist and diabetes nurse educator by calling (212) 420-3450. Patients can call to make an appointment on their own but we will need to contact your physician to get a completed referral form. Physicians can also make a referral directly to the office. For information about our staff, see our staff directory.

For Referring Physicians
Physicians can refer patients for diabetes education consultations with a nutritionist, exercise physiologist and diabetes nurse educator. For your convenience, here is a direct link to a PDF referral form download.

Please note that patients referred for these services will not be seen by an endocrinologist, but only by a nutritionist, nurse or exercise specialist.

For Patients

A Checklist for Your Visit

The following list will help you remember items and information to bring to your appointment:

  1. Bring in your glucometer (if you have one)
  2. Bring in all medications (or a list of them)
  3. Bring in your completed physician referral form (Initial consult only)
  4. If possible, bring in any recent blood work
